Monitor is considering changing the mechanism for reimbursing planned specialised care, so that it achieves better value for patients and the NHS.

Revising specialised services top-ups

This document explains the reasons why we have reviewed the policy on specialised top-ups and gives options for how we may implement them.

We have worked closely with the Specialised and Complex Care Advisory Group over the last 2 years during the development of our proposals.

The main reasons for our review include:

  • the shift to HRG4+ currency design
  • introduction of Prescribed Specialised Services following the Health and Social Care Act 2012
  • an independent review of specialised care by the University of York

Next steps

Monitor and NHS England originally proposed that these reforms would be included in the 2016/17 national tariff consultation. We now propose to consult on these reforms for implementation in 2017/18.
