Made it clear in the document which countries it applies to, fixed an error with the regular expression in row 1 of the file format, and applied some minor formatting changes.

'What data will be collected' - clarified that data should include appeals relating to all candidates taking the specified qualifications. 'When will the data collected' - updated the number of reporting deadlines for collection from 3 to 2. Updated the 'General completion guidelines' section to include guidance on appeals not to include, guidance on multiple appeal submissions, clarifying the 'NumberOfCandidates' field, clarifying the 'TypeOfDecisionAppealed' field and what to do if the effect of the qualification level grade is unknown at the data cut-off point.
