How to fill in the application form to add extra modules to your DVSA earned recognition accreditation

Download and save the application form to your computer or device

Fill in the application form.
You will need to:
- check section 4 of the DVSA earned recognition scheme guide
- meet all the eligibility criteria set out in section 1.4 of the guide
- read and understand the terms and conditions
You must answer all questions. Answer ‘N/A’ to any questions that are not applicable.
Your application may be refused or delayed if the Driver and Vehicle Standards Agency (DVSA) considers the required information has not been provided.
Questions
1. Which additional module are you applying for?
Select the additional module (AM) number from the list. Check the operator standards document if you are unsure about the correct AM number.
2. Are you a DVSA earned recognition scheme member?
If you are an existing DVSA earned recognition scheme member, you need to complete the entire application form.
If this is part of an application to join DVSA earned recognition, fill in sections A, B and F.
3. Name of entity or group
State the name of the entity, as stated on your operator’s licence, or the group applying for earned recognition. Include ‘trading as’ if applicable.
If you are an existing scheme member, enter your DVSA earned recognition number in the format 0000-0000.
4. Contact name and position
State the name of the person you wish us to contact in relation to this application. This should be a director, responsible person or transport manager.
5. Telephone number
State the contact number for the business and extension number of the contact if applicable.
6. Email
State the email address of the contact.
7. Contact address
State the address to which we should send any correspondence.
8. Do you wish to include all your operators licence numbers in the application?
You can apply for either all or at least one of your operators licences which must include all centres.
If yes
If you’re an existing scheme member, DVSA will use the details it currently holds.
For new applications, DVSA will use the details on the application form to join the scheme.
If no
Enter the operator licence numbers you want included for the additional module.
9. Are there any changes or planned changes that may affect your DVSA earned recognition status?
State ‘yes’ or ‘no’ as to whether there have been or will be any changes made to any of the licences stated in this application.
If ‘yes’, please give details.
10. Management IT system details
Confirm you have contacted your IT system provider to confirm the addition safety inspection items are included and are being measured as part of the M2 key performance indicator (KPI).
Check the audit standards for additional modules for details.
11. Have you already completed an additional module audit?
You can complete an additional module audit either:
- up to 3 months before you apply
- up to 3 months after you apply
If the audit has already been done, enter the date it was completed and attach a copy of the audit with the application. If DVSA considers the audit has not addressed the issues raised, it can request an additional directed audit to be completed. You will have to pay any extra audit costs.
If you have not had a periodic audit that includes either section 6 (security requirements) of the HGV standards or section 6 (security requirements) of the PSV standards, this must be completed at the same time as your additional module audit.
12. Name and address of audit provider
Please give the full details of your nominated audit provider.
Sending your application
To submit your application to DVSA:
- download a copy of the form
- save the form as a PDF
- fill in the form
- email the form to [email protected]
The subject title of the email needs to be ‘Additional module application’ with nothing else in it.
You need to attach any relevant documents to the email to support your application.
