Form for use by suppliers requesting that the Groceries Code Adjudicator arbitrate in a dispute with a retailer.

Suppliers seeking arbitration should also refer to the GCA Arbitration policy
Completed forms should be sent to [email protected] and served on the respondent in accordance with the CIArb Rules at the same time.
The form should:

set out the main elements of the dispute that will later form the basis of the statement of claim

summarise relevant events, give details of the alleged Code breach, and make reference to specific provisions of the Code and how they should be interpreted

give an indication of the loss suffered directly as a result of the alleged Code breach, together with details of any attempt to mitigate that loss, and the remedy sought
