Details

These forms are for practitioners and employers who want to apply for a licence to administer radioactive substances to people in diagnosis, therapy or research. There is a fee for employer applications. Details of the fees are available online.

All employer and practitioner licence applications must be submitted through the online portal.

Particular patient licence applications should be submitted by email to [email protected]

If you are a research sponsor, please read our information on how to apply for ARSAC approval for a research study.

Please note that applications submitted on the incorrect form will be returned.
