Use this form to apply for a replacement ADR certificate for your vehicle if you have lost the original, or need to change the owner or address.
There’s a different process to replace a lost, stolen or damaged dangerous goods (ADR) driver card.
When you cannot use this form
You cannot get a replacement certificate if:
- your vehicle’s ADR certificate has expired
- there has been a change to the vehicle’s ADR category
You will need to book and pass an ADR test.

Save the form on your computer or device.

Send the form to the Driver and Vehicle Standards Agency (DVSA) using the service to apply for a vehicle test or certificate for a lorry or trailer.

Pay the application fee by credit or debit card (DVSA will tell you how to do this after you’ve applied), or using a pre-funded account.
Your application will be delayed if you do not fill in the form correctly or pay the right fee.
