Details

Fill in this form before taking a dangerous goods (ADR) vehicle for a test. Take the completed form to the test centre and show it to the examiner before the test.

If the vehicle is not purged or is laden with dangerous goods it will need to be accompanied by a person with the correct ADR training certificate before it can be tested.
