If a medical referee asks for a post-mortem examination, you must complete this form afterwards.

The form enables you to give information about the cause of death.

It also asks you to confirm there’s no need for toxicological analysis or an inquest, so the body can be released for cremation.

This form should be used for deaths occurring in Scotland, Northern Ireland, the Channel Islands, or the Isle of Man, and where the cremation is to take place in England or Wales.
