Letters between the Chair of the independent Child Safeguarding Practice Review Panel, Secretary of State for Education and ministers at the Department for Health and Social Care (DHSC).

The panel’s letter announces a national child safeguarding practice review into sudden unexpected infant death (SUDI) in families where the children are considered at risk of harm. It also gives further details about the review including:
