Use this form to report significant incidents of percutaneous or mucocutaneous exposure.

What is a significant incident of exposure?

A significant exposure is a percutaneous or mucocutaneous exposure to blood or other body fluids from a source patient who is infected with:

  • HIV
  • hepatitis B surface antigen positive (HBsAg positive)
  • hepatitis C

When to report incidents

  • report all occupational exposure injuries with a risk of HIV, hepatitis B and/or hepatitis C infection
  • report all incidents where the healthcare worker has started post-exposure prophylaxis (PEP), and the source patient HIV status is not known
