Use this form to ask for a written record (also known as a transcript) of what was said at a hearing, trial or tribunal.

You can only apply for a free copy of the judge’s sentencing remarks if the defendant was found or pleaded guilty in a Crown court and you are:
- a family member of a victim of homicide (murder, manslaughter of offences of causing death on the road, such as dangerous driving)
- a victim of rape or serious sexual offences

To access and fill in this form on your computer you’ll need to use Adobe Acrobat Reader.
Follow these steps:
- Download Adobe Reader for free.
- Windows user - right-click on the form link then select ‘Save target as’ or ‘Save link as’.
- Mac users- right-click on the form link then select ‘Save linked file as’ or ‘Save link as’.
- Save the form (in your ‘documents’ folder, for example).
- Open Adobe Reader and then select the saved form.
