Before you fill in this memorandum of understanding (MOU) read the guidance on transporting assistance dogs by air to check if you must carry assistance dogs with their owners.

If you want to change the conditions of an existing MOU, you must fill in this form and send it to the Animal and Plant Health Agency (APHA).

APHA will confirm the MOU in writing.
