Details

These tools can help healthcare providers who are using paper-based patient records with collecting data for the tobacco and alcohol commissioning for quality and innovation (CQUIN) scheme. The use of these tools is optional.

The tobacco and alcohol CQUIN focuses on:

  • identifying patients who smoke and use alcohol above low risk levels to their health
  • providing brief advice and offering referrals to specialist services where needed

It reflects National Institute for Health and Care Excellence guidelines on the most effective ways of preventing ill health from tobacco and alcohol use.

You can use the data collection form to record a patient’s tobacco and alcohol use and the delivery of very brief advice. Then you can use the data collection tool to gather this data for reporting to NHS Digital. Or if you already have a way for recording tobacco and alcohol related information about patients, then you can use the data collection tool to help you with the case note audit.

Healthcare providers can also use this staff briefing document as a template to tell their staff about carrying out alcohol and tobacco screenings and giving brief advice. You can adapt it to fit your provider.
