Information request
I have been advised that I may be able to receive any information regarding work details (dates of employment and hours worked) and the name of benefits I was in receipt of (as well as the dates from and to) for the period dating 24/03/2012 until 23/03/2015 as requested on the form I have been asked to complete, ESA3 (IBRD).

The information you seek is not held by the Social Security Advisory Committee.
The Social Security Advisory Committee is an independent non-departmental public body sponsored by the Department for Work and Pensions, and has no access to information of this nature.
