This document is a user guide to the national taxonomy for incident learning in clinical imaging, MRI and nuclear medicine.

In 2024, the incident classification and pathway coding were refined and updated. The revised coding taxonomy now reflects the patient pathway from referral to clinical evaluation and has been expanded to include MRI and molecular radiotherapy.
