Template for animal health checks on farms to prove that animals used to make food, drink and agricultural products had annual health visits from a vet.

Details
Vets should use this template when visiting farmers whose livestock or animal products derived from their livestock will enter the food chain and may be exported to the EU.
Vets should:
- provide a copy of the completed declaration to the farmer
- advise the farmer they must keep a copy of the completed declaration for their records
- store a copy of the declaration on their veterinary practice clinical notes database
Providing evidence for exporting
Farmers will need to provide evidence of annual health visits if one or both of the following enter the food chain and may be exported to the EU:
- livestock
- animal products derived from livestock
The food chain information (FCI) documents for the livestock will need to include either a:
- vet attestation number (VAN) from a vet attestation visit or the funding to improve animal health and welfare service
- qualifying farm assurance scheme membership number
