Provider memo 24: Incorrect completion of ESF1420m forms (Initial)

Background
1. It has been brought to our attention that some ESF1420m forms (Initial) have been submitted with a signature date that does not match the actual date the Participant started on programme. The ESF1420m forms (Initial) must be signed at the start of provision.
2. The ESF Managing Authority and ESF Audit will deem ESF1420m forms (Initial) with incorrect dates as incomplete or incorrect and this could lead to financial penalties for providers.
3. Work and Health Programme provider guidance chapter 3 – ‘Acknowledging referrals, initial Participant engagement and registering a start’ paragraph 2, bullet 3 advises that ESF1420m forms (Initial) must be signed by the participant at the start date. The start date is the point at which the participant begins to engage with the programme. This is the face to face meeting between the provider and the participant.
Summary and action
4. With immediate effect you and your supply chain partners must ensure that the ESF1420m form (Initial) is completed in line with provider guidance. Any forms received after 25 November 2019 that include a start date that is different to the date the participant has signed the form will be rejected by the DWP ESF Admin team and returned to you.
5. There will be no corrective action required for forms received before this date.
6. Please review your procedures for form completion after 25 November 2019 and consider documenting any corrections to this issue; the dates this was undertaken and processes you put in place for resolution. This will protect Match providers in future audit checks.
