Use form TC689 to authorise someone (an 'intermediary') to act on your behalf for your tax credits and Child Benefit.

Details
Use form TC689 if you want an intermediary, such as Citizens Advice, to act on your behalf for your tax credits and Child Benefit.
Be aware that after an intermediary has been authorised they can make changes to your claims on your behalf, including changing your:
- address
- bank details
Do not use form TC689 to authorise a paid agent such as an accountant or other professional adviser to act on your behalf. You should use form 64-8 instead.
