Details

Use form TC689 if you want an intermediary, such as Citizens Advice, to act on your behalf for your tax credits and Child Benefit.

Be aware that after an intermediary has been authorised they can make changes to your claims on your behalf, including changing your:

  • address
  • bank details

Do not use form TC689 to authorise a paid agent such as an accountant or other professional adviser to act on your behalf. You should use form 64-8 instead.

You’ll need to fill in the form fully before you can print it. You cannot save a partly completed form so we suggest you gather all your information together before you begin to fill it in.
