Details

You can use this HMRC form to tell us:

  • you’re the personal representative of the estate
  • you want someone else to act for you, like a solicitor or accountant

A personal representative is responsible for finalising the affairs of someone who has died. This role can be:

  • an executor — named in the will to deal with the estate
  • an administrator (executor dative in Scotland) — appointed by the courts to administer the estate, usually if there’s no will

To fill in this form, you’ll need to print and sign it and include the National Insurance number of the person who has died.

If they did not have a National Insurance number, you’ll need to contact us by calling the Bereavement helpline.
